I'm a little puzzled with a transaction that took place from my qt-wallet.

What I don't understand is where this ~18.39 Dash amount came from. I sent 20 Dash to the address listed on Index item 1. I approved 20 Dash plus the small fee to be withdrawn from my qt-wallet. Definitely did NOT approve 38.39288666 DASH to be withdrawn and I've not set any payments out that were there previously and didn't go out.
The ~18.39 is sitting unredeemed according the transaction record. Have I lost it? Does anyone have any idea what is going on here? The ~38.39 definitely came out of my balance.

This 18,39 is in your change addresss which wallet creates each time you divide the one input amount....
If you send 50 coins out of your 100 coins you had in one address, if you will not point the exact change address you want (option in dash-qt) than the wallet creates it, it is the new address from your 1000 keys it already created when you created the wallet.dat for the first itme
