Posting you picture in the poloniex chatbox is what gave you away. The same picture which can be image searched to reveal your facebook profile. I actually assumed it was a false lead. Shocked

For being so confident in your opsec you failed miserably by posting your facebook profile picture in a chatbox where I was present. Then more or less confirmed it by stating it was the info you used to sign up at cryptsy.
