i spend my bitcoins. but only for digital goods.
e.g. my vpn accepts BTC. andi bought some apps with BTC.
I have used by Bitcoins to buy somethings but I like to pay with cash instead. Spending my Bitcions is something that I have to learn not to do any more because I can make allot of money with them.
So you are spending bitcoins by en cashing it to fiat and I think that are most people do it, since most of the countries have only few stores
that accepts bitcoin so we are forced to it, in exchange to fiat before spending it.
