www.Kraken.comFits your criteria quiet good. Have been using them for over 2 years now without an issue. Support responses is also really satisfying and fast as far as I experienced it. Bank Transfer was also really fast every time (SEPA).
Overall the exchange feels really secure. Liquidity is also really good on Euro/BTC pair. Liquidity is also okay for USD pair. If you want to use GBP I wouldn't recommend them since the spread is quiet big.
But overall, great exchange to just buy/sell Bitcoins on a regular base.
