Indeed, it is hard to use Bitcoin directly today, and you can't simply go to the nearest shop mall and just spend a few bitcoins there (but this may quickly change in the future).
If I am completely honest and look at the facts, then there is zero incentive from my side to actually spend any of my coins in a shopping mall.
1 ~ Bitcoin transaction fees simply make it not interesting as I pay for things instantly with cash or my bank card without any fees.
I would even add that spending bitcoins on everyday things like groceries and gas doesn't make any sense as such even if you had to pay no fees at all and all transactions would be instantly fast. Spending precious bitcoins on smaller and unimportant things is like spending gold coins and getting change in nickels. We already have fiat for this...

2 ~ The far majority of the merchants accepting Bitcoin are using a service such as BitPay where you require to have at least 1 confirmation when it comes to higher value transactions.
You could use a Bitcoin payment card, and then you wouldn't have to wait for any confirmations altogether
