leiyplane, you seem to have 2 if not 3 decoupling capacitors at the 12V input that burned out. Maybe faulty caps. Can I offer to buy your board for 2 BTC? Also, the people recommending you a higher wattage PSU are wrong. A 3-module non-overclocked Avalon draws only 500W @ DC (12V + 5V + 3.3V). A 650W PSU is plenty enough (~77% load).
