Yes, you should use the new Google Form.
We have received three new swap request by Google Form, the two requests already processed, the other one request cannot be processed because the file uploaded is expired, please upload to https://uploadfiles.io/My yesterday submission expired. Just submitted a new form with updated fresh URL for .dat file.
