Since any claims after 31 Oct will not be accepted.
As this bounty complaint form is targeted for those that submitted before 31st Oct and had issues receiving them.
I have already submitted my claim form before 31st Oct17 and I also have made post regarding confirmation of entering etherium address, I still did not receive any token
