I have the same problem but no fix yet.
Setup: asrock h81 pro btc mobo, 3 x gtx 1070, 4gb ram, cpu intel G1820

I tried hackflags, gen1, gen2 everything with a fresh w10 (1709) installation. But 1 of my 3 cards still have code 12.

Any tips left i can try? Im out of options
