Looks like some kind of hardware failure to me. When the fans on a graphics card spin up to 100% instantly it usually means that there's no connection between the card and the motherboard. So in your case the system "loses" the card as soon as you start mining. Not sure why yet, maybe something with the board or the psu. Did you try different pci-e slots?
