Re: ASRock H81 Pro will not accept more than 2 GPU's (Code 12)
"Code 12" appeared after I updated Windows 10 to version 1709. Earlier the rig worked on Build 1703 WITHOUT ANY PROBLEMS.

Reinstalled Build 1703 - all devices work without problems.
