Locked out. Asking for a password and no pass or email were set.

Support@primedice.com). All accounts are given a password as seen under "my stats" it will only prompt for it if cookies are deleted/device is changed.
I never set an email, so sending an email is just the same as sending a message here or adding a comment here. Anyone could send an email and no way to determine if it is really me. My fault for not noticing the password. Can you delete or disable the account and I will start a new one? Sorry for the inconvenience. Gotta love the internets.

If you use a new address you can get the bonus applied, I suggest generating a new address on whatever wallet you are using.
