How this cold storage works? Is it a chip card? In that case some kind of device needed to update data?
Or this associates with some wallet address? If so how it sync? Or is it a paper wallet?

It is a paper wallet. On the front there is an address and a QR code, making it easy to receive funds to that address. On the back there is a scratch off sticker that conceals the QR code containing the private key.
