Maybe the codes wasn't synced, have you tried going to settings --> Time correction for codes --> Sync now

But what do you mean by one code for all accounts? do you mean the backup code (16 characters) or the token which you are entering everytime you login? (6 characters)
I appreciate your response. Yes, sorry I forgot to mention but I did try time correction and syncing...that was not the issue either. The codes I'm referring to are the 6 digit tokens, not the 16 character backup code. Incidentally, the phone app logs me in automatically so I never needed the tokens to log in, which is why I only found out about the issue when I tried to change password (of course also needed for sending funds etc...).
