Had a quick look at your output .... I noticed that you have the default setting of 24 for "Miner Count" in your config. As you wrote, it is a 4 "blade" unit so the correct value would be 32.

Can you also please check the hardware addresses of your both units network interfaces, they need to be different.
A factory reset followed by a fresh firmware flash will set them to the same address.

You can also try:
a) disconnect 3 out four modules and check if you have faulty one (burned chip etc.) ... remember to set the miner count correctly!
b) run the modules in the working Avalon miner in order to check that they are OK.

If it turns out to be your FPGA board, I suggest you get a new one from the bitmine.ch guys ... Let them know I send you, they will FedEx you one for a reasonable price.

I would try pulling the jumper and trying to run it directly from a linux box
if it still dosnt work, you can count on a bad FPGA board.
