I in no way condone the way the NK establishment has apparently treat their citizens but what I would say is that the paranoia is probably justified given the USA carpet bombed their whole country and wiped out 30% of their total population. Only stopping the attack because there was nothing left of substance left to bomb...
Not only that, but then periodically conducted huge military exercises with the South (bombers included) which are basically a threat to repeat the feat (that on top of the verbal threats of invasion and regime change).
But most Americans don't know, or care, about any of that, and mainstream media isn't going help them there either.
