Firstly, it is cheaper than usual doctor appointment not because of poor quality. It costs less because the doctor does it in his spare time so costs (rent, insurance, some of taxes) are reduced.
Secondly, the doctor would examine patient health status. if the problem is serious and danger, the doctor would not waste the time to consult online, but recommends calling an ambulance. We focus mostly on the primary link of medical care
