You've asked to provide more info above. Those logs showed something went wrong (that's only the result) we need something that may cause that. As other people said above, it can be caused by various reasons:
Power Issue: then tell us PSUs that you are using
OverClock - Under Volt: then tell us your current OC-UV settings.
And so on..
Show us your bat file, hide your sensitive info! It will make us easier to analyze your problems.

23:57:34:677 185c Restarting OK, exit...
this occure whit two gpu's "3,5" rarely gpu 0
iuse claymore 11.9 with 18.5 amd driver. now actually i use claymore 11.5 with blockchain driver before, but this happens again .with one difference .its not restart claymore but stuck on this :
WATCHDOG: GPU 5 hangs in OpenCL call, exit
It showed that you've set (-minspeed), when your rig doesn't reach that specified amount of speed, the miner will restart nor call (-r) parameter (if -r set to "1") it will re-starting Claymore.
Try to bring back your cards setting (troubled card) to default may help.
Will be better to visit Claymore official thread for the best result related to your problem!
