I might be wrong to my answer however I will try the answers from the basics I understand.

a.  I guess it might be an automated process and there must be a default time period. Also looking at the email "acctcomp15", it has something to do with the 2015 hack.

a.  In this case, the original account owner might locked the account when they learnt that the account got hacked or it could be any mod or staff manually pushed a button after learning the account got hacked

a.  This one is self explaining. So I have no doubt that this is automated


- It looks like some message shows up for someone who appeared to fall into certain list of potentially hacked users (in other words was manually added to some group by mods and thus received the security block) - is that so?
a. Mod could have some special features to make their judgements so they may have a dedicated page to view a report.
- Modlog page often displays the "woke up" lines - are they connected with blocks somehow?
a. "Woke up" might be simply for just logging in after certain amount of time. I guess it works in general.
