Most of your suggestions if not all are already present in my script in one form or another, but...
I will only comment based on my own perspective, tungaqhd might have a very different line of thought.
Regarding the number of ad-spaces, its difficult to create a layout that supports both a gazillion ads and at the same time looks good with few ads.
Who do you cater to?
How much control should be left to the publisher?
And how should administration be set up?
And when does advanced features turn the script into overly complicated? Including a fully tricked out SQL database might be a little overkill...
I tried making my comments general in nature so not to derail the thread.
Tungaqhd > I couldnt resist laughing a little at how similar our positions are even though we have completely opposite approaches to our scripts. Were solving the same issues but in completely different ways. Im serious, were talking diametrically opposite coding methodology here. Lol.
But yes, speed is important, you can never have a too fast script.
