There have been several and different complaints on wallets that were hacked. Some of them were also able to tell some of the mistakes they made, while to some it was like dream to them.
Most times wallets get hacked as a result of exposure to phishing sites. Some have mistakenly entered their private keys into phishing sites and by so doing, their funds were stolen.
