**Do not contact our telegram admins, they cant help- you MUST submit the form.
Important Information: The form will be locked on December 24th. We will not accept any submissions after this date. Please be as detailed as possible. We will IGNORE any forms that will not be clear / will fail to provide all the information required. All the correction will be made AFTER the form is locked. Only then, we will publish a new and final spreadsheet.
