thanks for posting in the known alts thread, there's a form in the OP, https://bitcointalk.org/index.php?topic=2544574.0 if you copy that code to your post you can add the information into the relevant sections and your post will be uniform with everyone elses.  There's some tutorials in the second and third posts on how to archive information etc.
