if I've created account/wallet on coinbase or any other exchange then my account is hacked and my coins/money lost who are responsible for it

either u or coinbase, if u clicked something malicious before and most likely you had been hacked by someone from ur own action so you are the one who should get blamed. coinbase is a pretty much a safe exchange in terms of security, so i think you had done something bad before.
Indeed they will not let the public use platform like coinbase if hackers will just easily bypass every security it made so i think we should cross out the exchange where the OP created an account, if your account got hacked then you only have to blame yourself for your that and no one else but if something happens greater than that then neither you or coinbase should be blame but someone else that is responsible for it, remember that internet is a open network and be careful of your actions because with just one click someone can get into your personal info.
