if I've created account/wallet on coinbase or any other exchange then my account is hacked and my coins/money lost who are responsible for it

IMO.. if the account that you registered at one of the exchanges is hacked then the one who has the right to be responsible is you yourself because you are not maximally secure your account every transaction, but if many accounts on the exchange are hacked then the developer responsible is the exchange due to security they are very weak

and how i make my account safe  Huh
maximize the security provided by the exchange (enable the Authenticator feature), clean your laptop/computer regularly, do not carelessly install applications and secure your internet network.
