If everything you said is correct, then the most likely scenario is that your backup seed mnemonic (aka 12 words) was compromised.

Did you store your 12 words "digitally" ie. a screenshot or in a text/notepad/email message on your device or in the cloud? If so, that is most likely how it was compromised and your funds were stolen.
