@Timelord2067 I'll update my offline copy right now. Sounds good to me but isn't it redundant if we report here the connected alts along with his ban accounts and then post it again in this thread: https://bitcointalk.org/index.php?topic=5094661.0?
To @Timelord2067 please refrain from my reply above on why I didn't have to include the "Reporting" branch in this format. I'll just directly report this to mod within this link for them to look into.

Essentially, this thread is where you post the bulk of the details.  In the other threads or to CM/mods you post a very brief outline of who is banned with a link to this thread (i.e. your own post).

I mean in the "n Accounts Connected:" you colored them there red and that means they're ban and it's kind of reporting in its sense that they are really ban or doing ban evasion. What I do when I report ban evasion case/s is I directly linked it in this thread and it will be redundant if we post it again on that thread, hope you know what I mean. By the way, why not these on the note "(Note: Banned shown in red / Inactive in Blue / Active in default hyperlinked format)". Just a suggestion.

I like your solution to the following, I've tweaked it a little:

"(Note: Banned shown in red / Inactive in Blue / Active profile (in ordinary link colour))".

The Submission Form has been tweaked/updated with the above minor modification.

Kudos.
