if you still have access to your wallet used to generate the address... try
getaddressesbyaccount "" in the qt client console. you should see all generated address their and you can find yours... also once you get your coins from submitted shares... you'll see it appear in your wallet
My wallet was on VM and I save/copy a fresh VM to get new token so wallet is never saved.
If router and modem both are rebooted with new LAN/WAN/Wireless address of router which can be arbitrarily modified, the modem always get a new IP address in no time from ISP with dynamic IP address allocation.
