"gay propaganda" is a crime in Russia (under the guise of protecting children of course) so that might be a reason why we so often see some of our Russian friends throw it around as not only an insult but a threat of some sort. However it is no longer classified as a mental illness there, so not sure what doctor you are supposed to visit even if you travel to Russia to do so.
