Because Milan played against Inter and it ended up with a 1-1 result. Both sides were capable of winning taking their efforts into account but it didn't end that way. Napoli drew with Verona with a 1-1 result home too. But Napoli and Milan are still 7 points ahead of their nearest competitor.
Napoli and Milan has been on their best form and both are topping the league with equal points but Napoli has a higher goal difference. Drawing their last matches happened like a coincidence inter Milan wasn't an easy contender for Ac Milan neither was Verona for Napoli although many efforts were lost by all four teams but the game ended has it was supposed to end. The second penalty lost by inter deprived them their chance of taking the lead
