According to the poll, Chelsea and Liverpool are given even a lower chance than Manchester United for winning the championship. And this made me laugh really hard. 
I don't judge the people who voted for Manchester United at the start of the season. But I don't understand why the other people think that Manchester United can still make it. It is true that they are recovering slowly now but I don't see much chance for them to get even very close to the leader.

And I voted for Manchester City considering the current conditions of top teams in the league table. Chelsea are continuing to lose their form and getting away from the leader. Manchester City and Liverpool seem to be the teams that have the biggest chance to win the EPL title at the end. But I see Manchester City being one step ahead of Liverpool. I think we are going to watch them clash until the last weeks and Manchester City will be reaching the title.
It is not so much about form maybe when it comes to Chelsea. Some of the games they clearly dominated and won easily, but there were also a couple of games that were super close calls and Chelsea succeeded in the end with a good portion of luck. That luck seems to have left them for now. They are leaving points on the table week after week and City is punishing that brutally.
