This seems like a good start, i used an automatic tool from https://www.qrcodechimp.com/, and this is the result.
There is one small problem with this being used as a logo, because it's almost unusable in smaller size, and that is most important thing for logo, to work in all sizes (and colors).It can work partially, but qr code function won't work for sure.
The holy google said that the minimum size is 2x2 cm, i run a test with this size and didn't worked, but with a 4x4 cm image worked.
