Mostly any physical prize will be sent using personal name. I have experienced some when I was active in local casino long time ago when I was still single 
. I received some prizes like T-Shirt, Hoodie and my best reward was a Blackberry Bold if I remember it correctly. All the reward was sent from a personal name and I do even think that it was a fake name. At that time I did not care about my privacy as I was a college student (less experiences) so I had no worry to share my address to receive the prizes. Now it is completely different, I wont share my address for gambling related things even if the prize is a motorcycle or a car since it can be something dangerous for my job career.

Everyone who is not familiar with privacy at the beginning will do the same, may we will regrets about it now, but something already happens, we can't do anything except for the future.
