There's a problem with you google form to fill. Where instead of asking about bitcoin address you set it as an email so if we don't type as this form @xxx.xx for example it won't accept it you should change that.

And you didn't clarify how can we be accepted and when you going to pay users.
Participants who will fulfill the requirement will be accepted.
