This is what I see from your logs under ECHU[1 1 1]  according to Avalon description logs you may have a bad control board or hashboard have poor contact.

What I would like you to try is that try to replug all cables between the control board and the hashboard and make sure the cable on the hashboard is connected properly. If you have some extra cable you can easily know if the issue is the cables or if you have a multimeter you can try to test if the cable continuity is present.
