These are from the 3rd design in the series called "The M.O.A.B.", The Mother Of All Bubbles with a total mintage of 100 coins. Now there is a short backstory about the shmexy coin #69. The private keys in this series were handled by minerjones himself, and as a proof of that he added an extra "mantis"-hologram on top of the original hologram.
Once the coin was in the hands of the grader, he/she had thought that the extra holo is some sort of an artifact and removed it. After that the coin went through the grading process and ended up in a slab with grading, so it was too late to fix and was decided to be called an 'error'.
