Bad signs all around. It's likely that staff has been unpaid or underpaid for months. Who knows for how long the 2fa infrastructure had remained unmaintained for there to be failures for a simple withdrawal request. Huge failure on the part of such a service.

For the owners to not care about such fundamental issues I guess that they made their money and don't care anymore. Probably this site had long ago fallen off popularity and wasn't generating the expected revenues. It could have had a graceful shutdown that was fair though, shame to see it gone like that.
