Besides all the point you mentioned, I would like to add that over indulgence or spending more than what you can afford is what will hinder anyone from being rich, Even if you are ambitious and good at running a business or making a lot of money, if you are not living within your means, you will not go far and will always remain where you're standing, its not actually what you're making but how you keep what you are making that will make you rich.
Another one is gambling what you cannot afford to lose, I have seen one guy lose everything he worked for, in so many years in just one night of gambling, so even if you're making a lot of money, you are going to lose it fast if you are addicted to gambling, Your wealth accumulation depends so much on your character, so control yourself and do not overindulge.
