So instead of starting Bitcoin Core with "-testnet", you start it with "-testnet 000000000000000000011192c75d0a9cd5b7f11ed7cfa35f625d549b8aa093a7", after which it takes the hash of Bitcoin block 867462 as a starting point, and anyone else who uses the same startup options, will connect to this new testnet. There could be endless different testnets, and those ordinals spammers could happily create as many of their own testnets at they want.
