They tweaked the smart contract UI to display in the UI a different address (an approved one) while instead the funds were sent to another address.

And this would imply they are using some centralized service for their multisig setup since it affected all cosigners. I am sure this is an advanced attack but I have a hard time believing this was a real multisig setup.

Multisig doesn’t even exist for the shitcoin named Ethereum.
Multisig is where multiple signature are necessary to move some funds. Bitcoin does allow it.
The shitcoin ETH doesn’t allow it. So the single signature is split in multiple segment, and all those segment must be recombined in a single key to sign a transaction. This goes with very exotic names like MPC or SSS, but in the end is a single signature.
And no, probably they weren’t using an hardware wallet, but a proprietary solution based both on physical and on-chain techniques.
