Received this kind of bonus 1x only so far and it was juat after I levelled up to Plat3 several months ago. I cant really remember how much was the bonus but I think it was a nice bonus (more than my average weekly or even monthly bonus). What I know is that it was because I pushed myself for 1-2 weeks to level up so I've wagered more than I've used to on that 1-2 weeks while chasing a new level for me.
