Each time I tried to pay a service in real life directly with btc it has been crazy overpriced like +30%
I definitely give up and doesn't need even to mention anymore. unless you're buying something really expensive (like a yacht...) you can easily change a some satoshi and get cash. I would never pay a rent in btc (and personally I dislike questions about " how did you know btc"? Roll Eyes )
Nowadays it's full of services that allow this kind of payment... If a seller doesn't accept at a fair value btc this is not my issue...
I personally never encountered such crazy mark up, But I guess it does happen sometime knowing that some payment gateway charge too much fee for processing and that includes conversion fee, payment gateway fee, etc.

But if it's just sending bitcoin directly to the merchant like peer to peer. I don't think such mark up will happen otherwise the merchant is just being greedy.
