Bought new from amazon.com, includes original box and card. Mined with it for about two weeks at 74c, never overclocked. It pulls 550 kh/s using scrypt.

Yes, but you will be unable to change withdrawal address during the SFA session. Only GA sessions will provide access to this feature.
