everything has a purpose. saying bitcooin failed completely is a naive statement. blockchain still has a long way to go. The development happened in last 3 years in blockchain is much rapid as compared to previous years. may be next 3 years will be crucial.

you can try coinexchange. they have pretty lax terms for listing and they dont charge huge.
YoBit is another place you should apply. 0.1 eth for application, no listing fees. though it might take upto 2 months get lisetd.
iQuant  - low listing fees
exrates - listing fees is bit on higher side, but super fast listing

better place to look for is stm, warriorforum or copyhackers.

need more info. what you are asking is not token economy. number of tokens depends on your tokens utility, distribution mechanism, distribution channels, potential userbase, factors affecting the price, inbuild features that can help token price to sustain or rise.

apart from things already mentioned here, there are lot of other things that can be done. some might be little grey but highly effective. Most people have basic approach to getting traffic. traffic is not equal to paying customer or trading user. so tell me one usp that can be marketed to gain userbase, and whole campaign can be based around it. I'd also start buying ads on CMC, getting some fake volume. redesign the UI as per 2018 standards, and relaunch exchange saying you are upgrading the platform and atleast add 1-2 new features. Do some PR. this shud help you position in the market and give good burst of targeted traffic.

you can try coinpayments. google for crypto gateway . you will come across many. if you are on shopify, there are many apps for that

most icos focus more on show off while fundraising so they create hypothetical whitepapers without any backing. beside most of these icos have makeshift teams. sure to fail

i dont recall any such crypto bank yet, but i have seen some australian ICO aiming to do that/ for now you can do lending on bittrex or poloniex
