If the price of bitcoin doubled by then, the monetary reward in $USD would be the same when the reward is halved

What can cause a bitcoin transaction to fail to confirm?  I know having a transaction fee of 0 can result in miners not being willing to confirm your transaction.  What are some other possible causes?

Thanks guys.  I know the risk is extremely low in these instances.  Have a client who wants bitcoin POS integrated but doesnt quite understand how it all works.  Looking for the easiest way to explain to someone that is bitcoin illiterate that receiving multiple confirmations isnt really necessary.

How can you avoid having to wait the 10-30 minutes it takes for a transaction to confirm when accepting bitcoin for POS transactions?

Thanks for the response. Would an escrow service using three-sig transactions (ig. buyer, seller, and escrow service get key and 2 of the 3 keys can release funds.) help to protect against fraud, or is that how most bitcoin escrow services typically work anyways? 

If required to use an escrow service that you are unfamiliar with (untrusted) what stops the escrow service from taking the funds apart from the fact that their reputation and potential for future business will be ruined?
