It has resurfaced and my miner always hangs as soon as the mining starts and have to do manual reboot

It's hardware-related issue, drivers report wrong temperature, usually it means that this GPU is not stable. Check its riser, OC, PSU.

Right now i'm investigation on defective fans... It seems that its the cause in my particular case
It's not fans fault, I have tried putting the faulty GPU in another PC and it worked, but I'm on older Claymore miner on my other pcRight now i'm investigation on defective fans... It seems that its the cause in my particular case

and miner hanging, this is from a rig which was running stable for 2 years ++
on latest claymore

I have exactly the same problem ! Look above your post
Change the riser to new one. That will fix it.
