OMG... I have flashed ck 1.2.5 binary... it was without any changes after this... but after about 5 seconds I have turned it on again after flashing the chip near the LED7 fired up!

What is your miner version? If the rear lights are blinking, it's testing for the frequency. Depending on the modes, heavy testing or light, the time to init will take less than a minute and mainly 1 sec if you choose to run light test on them. Once the front light blinks, it means it's hashing...
The rear LEDs are all off now after flashing, only LED1 (power) rear led works.
There was 2 rear leds working before flashing (LED7 and LED8 I think)
Front LED always blinking now (2-3 time per second) even when the cgminer not started.

Switch to the latest versions of mining software, also check the firmware you used, there is another define (#define __PRODUCT_MODEL_JALAPENO__) which must be set for the Jalapeno line of products. If you have anything else defined it will probably upset it.
reflashed with this change (#define __PRODUCT_MODEL_JALAPENO__ ) - no result again

tell us more what kind jtag did you use and what kind firmware file you use .hex or bin?
upgraded cgminer to 3.4.0 - no any changes.
I have used AVR Dragon, compiled the firmware with Atmel Studio

The red light is flashing on/off 2-3 times a second. When I hook up JTAG (AVR DRAGON) it flashed quietly like normal and I can still re-program it.
I flashed with the untouched firmware 1.2.5. Anyone know whats wrong? :/
EDIT: Alright, after very quick trial and error, this has to do with the "#define __ASIC_FREQUENCY_ACTUAL_INDEX 7".
Apparently 7 doesn't work on mine. I got 5 working fine, will try and adjust with 6-8-9

I have the same problem with my Jalapeno.
I have flashed it with default __ASIC_FREQUENCY_ACTUAL_INDEX=7
and got this problem.
Reflashed with __ASIC_FREQUENCY_ACTUAL_INDEX 5 - same problem again...
with __ASIC_FREQUENCY_ACTUAL_INDEX 1 - same problem.
Also after upgrade there is only one big LED4 blinking and LED1 (power), but before upgrade there 2 LEDs on the other side of board was on too...
How can I get it fixed?

Just so you guys know, I set out to create a new guide for v11.10 (Oneiric Ocelot)... but after spending a better part of whole day trying to get it to work properly with multiple GPUs, I have given up for the time being.
v11.10 is just plain not good for mining for a number of reasons if you have more than 1 GPU (it's ok for 1 GPU though).
I am going to re-do the v11.04 guide and remove the x32 version, only doing an x64. I'm pretty ticked off I wasted a whole day on v11.10 and I'm pretty ticked off at both AMD for not fixing their crap ass drivers for 3+ revisions and at Ubuntu for foisting the ultra crap that Unity is on us and also for using the crap AMD drivers that are a bag of fail (v11.
as their base fglrx install.
Look for the new guide soon.

But drivers before 11.7 not work with kernel 3.0+
So I have found a patch:

and using 3.1 kernel and 11.6 drivers with this patch on Ubuntu oneiric.
Everything works fine (also spent full day to found this solution).
