I think it all about media effect and political issue
first, it is too little news about how good of bitcoin. most news about bitcoin are bad.
second, Bitcoin is a innovation idea, and it keep improve day by day. from just digital asset (BTCtrade) to debit card(like ANX)
not so many people understand and keep track the update.
third, government should have control on money. if country give money control to other like bitcoin
it is quite dangerous itself

Because i think this is usage of Bitcoin debit card
high fee than normal debit card / credit card
In normal user view, it is no different between these card.(exclude some traveler and multi-national company)
So many bank wouldn't sell it, it wasting their time.
and the Bitcoin company wouldn't sell it normal use, the market is too little

As,i know, ANX intl, bitpay, coinbase are doing these project....but it so far alway to be normal debit card.

I think more competition. will be lead price down. but i think most user of these debit card are people who are multi national. the exchange rate is higher than the debit card fee are lot

I think the different is between what company focus on. bitcoin company can have many focus.

The last you mention is quite special

i think bitcoin do not make bank disappear but it make company to be a bank more easy.
for example, ANX in Hong Kong provide debit card and bitcoin saving. in my view it like bank
