I think that would be depend on the country you belong. Due to not all country had the same way of accepting the concepts of bitcoin. Like here in my place we had rebit.ph where we can cash out our bitcoin anytime we want.

In my country, I use a XAPO card to cash out very quickly. I know they have a fee for each withdrawal, but it is more convenient using this card to cash out whenever I need money. I think to convert bitcoins to fiat all services will have some fee but as you said some may charge smaller fee. But I find it so far XAPO is good for me need.

although i am using xapo wallet, but we do not have the facility to use xapo card in my country. i think they do not have service in our county, but i am using urdubit. i think it is one of the best service in our country, it provide us a very good service i can cash out my money with in 4 working hours, for every time i found their service as good and convenient.
