It will be out of sync until you download the entire blockchain. If you scroll over the bar you can actively see if it is downloading blocks and how many weeks left. You have to wait until you download the block in which your game transaction was in to see it "credit" your wallet.

Yes you must have an internet connection while the blockchain syncs.

No it generally takes hours. The longest I've ever heard it take is like 24-36 hours for someone with slow internet. I loaded the wallet in a few hours.
